These are the steps to performing a good refraction:
- Determine the BCVA either from the patients history, pin hole, or current form of correction.
- First refine the power. For visual acuity 20/40 to 20/80, use 1/2 diopter changes to question better 1 or 2. For 20/30 and better, use single 1/4 diopter clicks. Avoid adding minus. This is especially true for patients over 40.For every phoropter click of improvement, lower the visual acuity line on the snellen chart.
- Next, let's address any astigmatism present. This is easiest to refine at 20/30 or better visual acuity. First increase the line size on the snellen. For example, if you refined the power to 20/25 then return to 20/30. Move the "JCC" or the jackson cross cylinder in front of the patient. Turn the axis wheel to 180 degrees and flip the JCC. If the patient finds improved visual clarity with the dots at 180, turn the cylinder knob. If the patient finds improved visual clarity with the dots at 90, turn the axis wheel to 90 and then turn the cylinder knob.
